Submitting the form does not constitute entering into an agreement. After verifying the submitted information and documents, we will prepare the agreement and inform you about the next steps.

Selection of one of our locations
In this field, specify under which of our addresses you want to register your company. Depending on your choice, the contract will be signed with a different operator.
For the Warsaw address, it will be Dime Office Sp. z o.o., and for the Poznań address – Krzysztof Kędziora Consulting.

Selection of a package and billing plan
Select the service package and billing plan you want to use. If you have previously made a selection in the Offer section, the form will automatically indicate the appropriate option. If you are still unsure which package or plan to choose, review the detailed descriptions in the Our Offer section.
Selection of additional services
Select the additional services you would like to use. If a service is already included in your selected package, you will see the relevant information, and the field will remain inactive.
The list does not include all the additional services we offer, only those worth considering at the start of your cooperation with us. Other services can be ordered during the course of the agreement.

Selection of the legal form of your business

Commercial company, foundation, or association - Any entity entered in the KRS register.

Entity in organization - An entity that is required to be registered in the KRS but does not yet have such registration.

Civil partnership - Select if you run a business in the form of a civil partnership.

Sole proprietorship - Select if you run a business registered in CEiDG.

Individual - Select if you do not run a business and would like to use our services privately.
Company Name
Enter your company name along with its legal form (e.g., Ltd., S.K.A., etc.).
If you run a business, enter its full name, including your first and last name.
If you want to sign the agreement as an individual not conducting business, provide only your first and last name.
VAT Identification Number (NIP)
Enter your company's VAT identification number (NIP). For entities in formation or individuals not conducting business, leave this field blank.
Persons signing the agreement

Commercial company, foundation, or association - Specify who will sign the agreement according to the entry in the KRS and the representation method. If there have been changes in the company and the KRS entry is outdated, provide the new details.

Entity in organization - Specify who will sign the agreement according to the founding document of the entity and the representation method.

Civil partnership - Specify which of the partners will sign the agreement.

Sole proprietorship - Leave blank.

Individual - Leave blank.
If the agreement will be signed by a proxy, specify their first and last name and add a note that the proxy is not listed in the register. (The note "proxy" is sufficient).
Enter your email address. We will send the following to this address:
1. Information regarding the agreement and the agreement itself.
2. Notifications about received mail.
3. Invoices and other documents related to billing.
After signing the agreement, you will be able to add additional email addresses, e.g., for mail notifications.
Phone Number
Provide your phone number so we can contact you regarding urgent matters.
Mailing Address
Provide your mailing address, where we can send you information if necessary. This can be your private address or another address where you will receive our correspondence.
If you ask us to forward the collected mail, we will use this address by default.
Your mailing address will not be visible in the agreement, so you will not be required to disclose it to the Tax Office.
Selection of shipments to be collected
We recommend leaving the default settings selected. However, if you need us to handle incoming mail more selectively, you can specify how we should proceed with different types of shipments. After changing the selection, information will appear showing how the change will affect the handling of each type of shipment.
We recommend submitting a postal power of attorney.
Submitting a postal power of attorney costs 26 PLN net, and we can submit it on your behalf. Submitting it at the post office provides several benefits, primarily:
1. It ensures continuity in mail reception in case of changes in postal law.
2. It allows for more effective complaints handling at Poczta Polska facilities.
Indicating additional persons authorized to collect correspondence
You can designate additional persons who will have the right to collect the accumulated correspondence from our office.
Persons authorized to collect without additional power of attorney are:

For entities required to be registered in the KRS - members of the board and proxies.

For civil partnerships - each of the partners.

For individuals running a sole proprietorship and those not conducting business - only the person with whom we have entered into the agreement.
If you want to authorize other persons to collect the correspondence, select the appropriate checkbox and enter their first and last names. You will be able to change your selection at any time during the term of the agreement.
